If you change your address, name, home or mobile telephone number, please let us know so that we can update your records. For change of name due to marriage or deed poll, you’ll need pop in so we can take a copy of your certificate too.
If you have moved outside our practice catchment area, you will need to register with a new GP surgery closer to your new address.
