Patient Registration

registrationNew patients are required to participate in a new patient examination on registering with the practice. In addition new patients will be asked to complete a simple questionnaire that will be used to provide us with medical history pending receipt of full medical records from a previous doctor. An appointment will be made for new patients to see the practice nurse at mutually convenient time.

Practice Boundary

When a patient moves outside the designated practice boundary notification of change of address should be given as soon as possible. This is very important to ensure that hospital appointments, correspondence etc does not go astray.

The practice will then inform the Primary Care Trust of this change of address and request that the patient is removed from our list. At the same time the patient will also be informed of a need to change to a doctor more local to their new address. A map outlining the area included in the practice boundary is available.
